The master of any vessel approaching the Port of Liverpool, or Port of Garston, from a foreign port must complete and submit a Maritime Declaration of Health.
Even in there are no suspected or confirmed cases of infectious diseases or death on board the vessel or aircraft, a Maritime Declaration of Health must be completed and submitted to us with all other ship arrival documents.
A Maritime Declaration of Health must be submitted with along with all other ship arrival documents using our upload ship arrival documents form.
